First: Find your group’s WSO number.
Requests cannot be processed without a WSO number.
Second: Submit your form
Questions? Contact the Insurance Coordinator. The Insurance Coordinator is the only point of contact. Do not contact the insurance company.
Whenever possible, please submit this form 3 weeks in advance of any deadlines and allow at least 10 business days for a response.
After you click “Finish” this form will go directly to the Insurance Coordinator and be provided to the insurance company. You will have the option of downloading a copy of the completed form for your records. Please send any questions and comments to the Insurance Coordinator by email at insurance@scws-al-anon.org.
Policy Renewal Update
- Certificates: If your facility previously required a Certificate, a new Certificate will automatically be emailed to the facility. Please make sure we have the facility’s correct email address.
- Rental Agreements: If your facility previously required a Rental Agreement (see Checklist Item #5), it must be valid into 2027. A Certificate cannot be issued until an expired Agreement is updated.
Complete the information below only if:
- Your facility is making a new request, or
- You need to update your facility contact information.
