Questions? Contact the Insurance Coordinator. The Insurance Coordinator is the only point of contact. Do not contact the insurance company.
Whenever possible, please submit this form 3 weeks in advance of any deadlines and allow at least 10 business days for a response.
After you click “Finish” this form will go directly to the Insurance Coordinator and be provided to the insurance company. You will have the option of downloading a copy of the completed form for your records. Please send any questions and comments to the Insurance Coordinator by email at insurance@scws-al-anon.org.
