Alateen Requirements and Forms
The Alateen Registration/Group Records Change Form (Mail-in) form is sent to the Area Alateen Process Person via the Contact Us page or to the mailing address listed below. This is a fillable online form and you will receive a copy in email.
If you are interested in becoming an AMIAS, you can go to an AMIAS certification workshop or to recertify, you can attend an AMIAS recertification workshop.
These forms are filled out online and submitted to the Alateen Events Coordinator email, you will receive a copy in your email after submission.
If you choose to print the form and mail it, you can send it the SCWS mailing address, please be advised that response times to regular mail may take several days.
